Lady McCartney, 38, formerly the model Heather Mills, accused the singer-songwriter of being violent, abusive and frequently drunk or high on drugs.
The claims, set out in documents leaked to a newspaper, allege that on separate occasions he stabbed her in the arm with a broken wine glass, shoved her over a coffee table and grabbed her throat during a row.
The allegations are said to be made in papers lodged by Lady McCartney with the High Court in response to Sir Paul's divorce petition.
The 13 pages are said to have been leaked to a newspaper.
Lady McCartney, who was today taking part in a TV broadcast at St Thomas' hospital, also refused to comment on the leaked papers but Phil Hall, her spokesman, said: "I am certain that Heather would not commit perjury."
The court papers are her response to his petition for divorce in July when he cited her unreasonable behaviour.
The allegations come amid the battle for his 825 million fortune and mark an escalation in hostilities between the couple.
Mishcon de Reya, the law firm that acts for Ms Mills, would not comment on the documents' authenticity but said in a statement: "The law firm can confirm that Lady Heather Mills McCartney stands by everything that has been filed at court on her behalf and intends to prove its truth in due course, should this be necessary."
McCartney is represented by Fiona Shackleton, one of Britain's toughest divorce lawyers, who refused to comment.
